BN100/BN70 Issues

We have a regular number of employees losing benefits because of a reduction in hours. The BN100 is ending the employee's benefits correctly, but not creating a BN70 for the employee. We had 5 employees who lost benefits due to the same PA on 5/29/11, 4 had the BN70 created and one didn't. On 5/27, we had two lose benefits, one had the BN70 created and one didnt. It is the same personnel action, and in many cases, the same benefit plans. When I review the BN100 it shows the health and dental plans being stopped effective the same date as the other employees. None of the employees have prior BN70 records. So I am not sure why one is getting the BN70 created and one is not!!! Any thoughts??????????????????????

There aren't any messages on the BN100 error report for the one who had the problem?

Probably asking the obvious again, but how about the plan termination rules on BN16.4? Plan missing, employee group missing, employees in question are in more than one BN16.4 defined employee group (if so, won't work), plan/group not defined as a Cobra occurrence, etc.?

All of those things are set up fine. No problem with asking, I would rather you ask what "seems obvious" to you....just in case I am missing something!
