Set up ESS Benefits Enrollment for a Medical Plan Spousal Surcharge

    Hi, we use Employee Self-Service for our Annual and New Hire benefits enrollment. I am trying to figure out if possible how to configure S3 ESS Benefits Enrollment to handle a Spousal Surcharge for our Medical plan. We also would like to add a new form/screen to our ESS Benefits Enrollment process that would display Spousal Surcharge Affidavit verbiage and have the employee select either a Yes or No on the affidavit which would update the HR11 Ben Flags->Spouse Coverage Health Coverage field/flag. Is there anyone that has done this?

      We added a custom HTML/Javascript page to capture spouse employement data and update HR11. We have a surcharge but don't have it tied to a medical plan, its just a deduction. We have a couple of backend Crystal reports used to audit it. After annual benefits enrollment, we use the PR514 to load the next plan year's deductions.
