**Form Purpose
Use Employee Tests (PA49.1) to
- schedule a test,
- record test results, and
- track follow up information for an employee.
After you select Completed in the Test Status field, follow up or renewal
information can be automatically calculated or entered.

**Form Purpose
Use Employee Test Eligibility Override (PA49.2) to
- allow an employee to be scheduled for a test for which the employee is
ineligible and
- prevent an employee from being scheduled for a test for which the employee
is eligible.
