PA92.1 - Incident Establishment

**Form Purpose
Use Incident Establishment (PA92.1) to define incident establishment
information.

**Processing Effect
Incident Establishment is a required field ieven if the organization
represents a single OSHA reporting establishment.

The population of an incident establishment is be based on process level,
department, and/or location code. PA92.1 enables OSHA Log, Summary Report
(PA490) automatically determine the annual average number of employees and
total hours worked within each establishment.

Note Changes to process level, department and location code should be logged
to history. This enables the application to make the most accurate
determination of average number of employees in an establishment over the
reporting year.
