PA212 - Emergency Contact Listing

Run PA212 (Emergency Contact Listing) to list emergency contacts for
employees from PA12.1 (Emergency Contact). The list shows for each employee:
the emergency contact's name, address, relationship to the employee, and home
and work telephone numbers.
