BS04.1 - Benefits Number Of Dependents

**Form Purpose
Use SEA Benefits Number Of Dependents (BS04.1) to define the number of
dependents an employee can add for each coverage option define on BN17.1
(Coverage Options by Plan).

**More Information
If you do not define these rules, an employee can add an unlimited number of
dependents.
