PA490 - OSHA Log, Summary Report

**Form Purpose Run OSHA Log, Summary Report (PA490) to print an OSHA 300 or
OSHA 300A report. Both reports print every time PA490 is run, even if no
incidents exist on Health, Safety Incident (PA90.1). You can run PA490 for a
particular population or date range.
