PA290 - Health and Safety Incident Listing

**Form Purpose
Run Health and Safety Incident Listing (PA290) to print a list of health and
safety incident information.

You can list the information for an employee or for an incident and create a
CSV (comma-separated value) file that extracts all the data from OSHA records
to import into another system.

You can also produce reports consistent with the new OSHA 301 forms.
