Receive the following errors in ladb.log regularly. Cannot determine the cause. Event log on app server and db server show no hardware errors.

Yes, the database is on separate server -- a virtualized server (VMWARE)
Yes, the MDAC version is the same on both servers -- 2.82.3959.
Yes, the application server has the client configured to use TCP/IP.
