Group Name Contains the employee group for which
the coverage record applies.
If the group is blank, the system will
use this coverage record for all
employees who do not qualify for
coverage for a specific group.

Start Date Contains the date the coverage
parameters begin.

Calculation Type Defines how the coverage amount will be
determined for the plan.
F = Flat Amount
M = Multiple Salary
P = Percent
S = Supplemental
X = User Exit
CVR-LIFE-ADD-FLG Alpha 1 BN17.3
Covered Individual Contains a value that determines who is
covered under the plan.
Employee is not a valid value for
Dependent Life plans. Spouse, Dependent,
and Both Spouse and Dependent are not
valid values for Employee Life plans.

Student Age Contains the age coverage terminates
for dependents who are flagged as
students. If the dependent is a student,
this age overrides the dependent age.

spouses and dependents, this field
contains the age dependent coverage
terminates for dependents if the
dependent is not flagged as a student.

Flat Amt 1 For a plan with a calculation type of
flat amount, this is the flat amount of
coverage. If a plan covers both spouses
and dependents, this is the spouse
coverage amount.

and the plan covers both spouses and
dependents, this is the dependent
coverage amount.

supplemental, this field defines the
increments of coverage an employee may
elect. Coverage under this plan will be
a multiple of this amount.

Multiple of Salary If the coverage calculation type is a
multiple of salary, this field defines
the valid multiples the employee may
elect. Coverage under this plan will be
a multiple of this increment.

Percent of Salary If the coverage calculation type is a
percent, this the percent of the
employee's salary which will be used to
calculate coverage.

Salary Type If the coverage calculation type is a
percent of salary, a multiple of salary,
or if the coverage limit is a multiple
of salary, this field defines which
field is used to define the salary to be
used in calculations.

Salary Date If the coverage is based on the
employee's salary, and the salary is
determined on a specific month and day,
this field contains the date the salary
is determined.

Sal First Mo If coverage is based on salary, and the
salary should be the salary in effect on
the first of the month for which a
benefit is created, the salary will be
flagged as first of month.
The As Of date is defined in the Salary
Date and Salary Year fields. The Flex
Calc date is the same as the date on
which flex dollars are calculated.

Rounding Method If coverage is based on salary, this
field defines how the salary amount
should be rounded.
H = High
L = Low
N = No Rounding
R = Normal Rounding

Before/After If the coverage amount is based on
salary this field defines whether the
salary amount should be rounded before
the coverage amount is calculated, or if
the coverage amount should be rounded
after the calculation is done.
B = Before

Round To If the salary amount is rounded, this
defines to what place the amount is
rounded.

Min Cover If a plan has a minimum coverage
amount, the minimum is contained in this
field. If the plan is a multiple of
salary or a percent, and the calculated
coverage amount is less than the
minimum, the minimum will be used. If
the plan is a supplemental plan and the
elected amount is less than the minimum,
the system will generate an error.

Maximum If the plan has a maximum coverage
amount, the maximum is specified in this
field. If the plan is a multiple of
salary or a percent, and the calculated
amount is greater than the maximum, the
maximum amount will be used. If the
plan is a supplemental plan, and the
employee elects more than the maximum,
the system will generate an error.

Min Multiple If the plan is a multiple of salary, or
if the plan has a minimum which is
specified a multiple of salary, this is
the minimum multiple.

multiple of salary, this field defines
the maximum multiple.

Rate Table If the plan specifies a reduction of
coverage at specific ages, this is the
rate table used to define the coverage
reduction parameters.
The table is defined in the Rate Table
Header file which uses the Coverage
Start Date and the Rate Table fields to
connect the records.

Age Date If the plan has a coverage reduction
table based on age, and the age is
determined on a specific date, the
employee or spouse's age on this date is
calculated.

spouse's age should be calculated for
the first of each month, first of month
will be yes.

employees who do not elect a coverage
amount. This is used for supplemental
